HI there.

My company the AMSN requires a certificate of attendance for people that attend our seminars and workshops.

The AMSN provides high quality CPD ( Continuing professional development) for Doctors, Chiropractors, Physiotherapists etc.

We require an attendance certificate that we can mail merge the attendees names into that gets mailed out into an A4 certificate that would be framed on their wall.

In detail it would need to be

* Appear very professional and medical. Something a Dr would be proud to hang on their wall
* Be editable by us so that we can include some brief detail about the topics the seminar discussed and the number of points recorded.
* ideally be made or at least saved in Microsoft publisher so we can edit the appearance ourselves
* reflect our logo's and themes on our website and mailing lists
* easlily allow us to mail merge in the persons name and title, and also show the name of the event and description of the event.
* use the blue and yellow color theme that we have selected for our logo's
* incorporate a seal of certification ( see attatched file of an idea)
* incorporate the signature and title of our CEO Dr John Stevens ( see signature attached)

sorry for not mentioning it earlier. I would prefer a portrait view rather than a landscape A4 layout. I think a cleaner white background would be better as well. It would be great if we can get the certificates printed around the corner and then mail merge the attendees name. I would prefer a less busy cleaner design. Hope that helps

I also need some space for some brief text to describe the event. Such as " This Seminar covered the latest developments in the understanding and treatment of Chronic Pain and the attendee has been awarded 7 formal CPD hours "
