I need a small 4-color banner for a print advertisement that measures

Banner Size: 3.25" x 3"

Included in the advertisement banner shall be:
1. Product image: MMKO.jpg
2. Product Price: $429.95
3. Website Address: www.sci-supply.com
4. Logo (small) (color or black and white) *Can place logo on product image to save space: logo.jpg

We want something with eye catching colors. We would like a high resolution PDF file with images at 300 dpi and all fonts.
