Contest Brief

We need a simple but elegant looking design for a brochure/letter, which is a customer agreement text. Its for a Forex Trading company so it has to look professional. Format to be Indesign, Photoshop or Illustrator. Source is required.

Requirements:
1. We need a very simple Cover (Logo in them middle, some nice faded background, like a curve, or particles, and the title on top of the page: "Customer Account Agreement". Color scheme to be RED, Gray, Black
2. Back Cover will simply have the logo in the footer, but smaller & a text under it, I will replace later so just put a text holder.
3. Subpages will have the attached text to fit in 6 pages. Size to be 11, with some formatting, like Bold & little bigger headers, bullets, etc
4. A page for the client to fill an APPLICATION (Last page in the Doc file. It has to be well formatted like the Sample PDF Form)
