I need a product information insert for a cutting board I'm getting ready to sell. The size needs to be 15cm x 20cm and the format needs to be PDF. I have attached a document that contains information in two sections. The first section contains information I want detailed on the insert, but you may change the words, if need you like as long as the meaning remains the same. The second section contains the following items that are required:

UPC (This needs to remain scannable)
"Made in China" text
Company logo
"Not recommended for dishwasher" warning

Additionally, royalty-free graphics are welcome to be added.
