Contest Brief

I am searching for a "Customer Feedback" form to be used in a restaurant. When any customer finish his dinner or lunch in a restaurant, waiter can present this form to him.
The basic purpose is to get customer data and feedback . But it should be such a creative that a customer can fill it happily & easily.
Most of the time customer avoid to provide any information. But this design should attract them to fill it.

Ask customer about: Quality of food, Service Efficiency, Cleanliness

Other comments/suggestions:

Please use catchy words to attract customers.

In Two rows ask customer basic information e.g. Name, DOB, Email Address

Please review us on YELP, we will serve a complimentary Drink and Appetizer on your next visit.

Note: At the moment , you may use logo of any famous restaurant. Later we will remove it with ours. Please focus on branding promotion concept.

I can create something nice, however I'd suggest that you don't ask customers for DoB and email address etc. feedback forms should be as anonymous as possible. It is recommended that they have to write as little as possible, but are also provided with a text area for optional additional comments. Just my suggestion, but the customer (you) is always right. will whip up something. Are there any particular color schemes to incorporate in the design?
