I need someone to design a reference form for my organization. I have attached the information that must be on the reference form and the way that the form should be laid out.

Please note that the contest has been posted for 30 days but a winner will be chosen immediately a suitable entry is found

Please note important information for the design

1) Please use my small logo as watermark for the form

2) Please place my main logo (with words and icon) on the form - upper right hand corner

3) The form must FILLABLE and be in PDF.

4) I want to be able to email the form to someone and they can click it and fill the form online. Then they can save it and email it back to me.

5) The information on the form must be saveable. In other words, i should be able to save the completed form with all information.

6) For the YES and NO questions, please put boxes in front of each option and people should be able to click the box

7) Please put a block of rectangle around each question and a block of rectangle around each comment question so that people can type within the box

8) I want the form to be a maximum of 2 pages

c'mon - editable pdf? that's no respect to partners to get references - I bet they won't use it in vast majority, as it is not convenient to use. make form on your site.

  Even on mac os x there's no software to edit pdf by default, and almost same statistics on other OSes. So if the goal to collect less references, then they move to it in very successful way.
