I need an order form with 4 Products ( Polo, Sweatshirt, Sweatpants, Long Sleeve T-shirt ). I have attached an example and another example of what i need at the bottom of the flyer which is the Order and Delivery Options.

wow.... nice one...nice copying of ideas and colors ismathsyle.. and nice one CH for the changes you wanted to make and making the designer expect for something.. now i wont be surprise .. for $10... ?!?! minus the fee for the site... thank you for wasting my time and effort.
