I need a designer for only one page for a wordpress site www.diaspo-cc.com. This page should be used for address book. Following functionality will be available on the page (Functionality implementation is not in scope here - ONLY DESIGN):

- Info to save in address book: Name (First + Last Name), Phone Number, Country, City, e-mail
- User can add/edit/delete each entry
- User should be able to start a transfer operation from address book by click on a button (symbol/icon)
- This button will popup a window for the user to enter the a value
