I need an editable prescription pad designed for Doctors and Dentists to prescribe our product.
Size needs to be around 100 mm x 100 mm
Denta-Med logo to be the header which is most prominent.
Peter Mac logo with the following text must be included "ENDORSED BY PETER MAC DENTAL ONCOLOGY UNIT AS THE PREFERRED DENTAL HYGIENE AGENT - BOTH PREVENTATIVE AND THERAPEUTIC" The Peter Mac logo must not be changed and the text needs to be editable as there is a specific font that must be used (DIN Next LT Pro). (See attached style guide.)
A tick box with space to add instructions in the Doctors own hand writing is needed eg; brush teeth and gums 3 x daily.
A space for the Doctor / Dentists clinic address stamp with "Clinic details" in faded text.

Hi. No we cannot provide it as it has to be downloaded and it cost us approx $60 AUD. That is why we need the design to be editable. The font is in the style guide document that I provided. Does this make sense? Regards Nick
