Contest Brief

I need a Trifold brochure design made for mailing
I need a professional eye catching brochure designed for my company.
The 3 outside panels will have company information and mailing address page.
The 3 inside panels will be used for an application form.
Ideally I would like the existing design theme of my website to show on the flyer.

1st page:
The main front panel will have a cell phone, (ideally a Samsung) with my mobile website showing on it. See attached screenshot for mobile website.
I want my Logo to appear on the top of the page.
I want my website URL also shown somewhere on 1st page.
Also possibly mention along the bottom: The Free Mobile APP that Every Driver should have on their phone!

2nd page:
When trifold flyer is opened the next page will show similar information for advertising your business as you can see on the page of my website entitled :get listed"

Back page:
This will be the page to use for mailing so it will have my address on top left corner (if looking at back page long wise)
Also along bottom of page would like to see something like:
(A small picture of our phone with mobile app) The Free Mobile APP that Every Driver should have on their phone!

Inside: Well designed application form in style similar to that on our website with all same information requires, and also with mailing and payment information at bottom of page as can be seen on our OLD application form (attached).

  @carlosov, Sealed may bring some designers that otherwise wouldn't jump in but in past experience I haven's seen large benefit of sealing contests. Where as when it is open and designers see which designs get stars there is more emphasis on what I like in the design and better end result.
