Contest Brief

I need a landing page to include the following:

1. A place for a video top, middle
2. Followed by some large text "Help us choose a logo and win one month of free Shaving Supplies"
3. 10 (ten) spots for 10 logos for people to choose from.
4. After people watch the video and choose a logo they should be presented with a form

Form Fields:
1. Full Name
2. Street Address, City, State, Zip Code.
3. Email address
4. Confirm Email Address
5. Phone number

After customer picks a logo and fills the form, they should see a message "Please like our page on Facebook to be the first to know our news and promotions."

Please provide a screen shot or a mockup of the page so we can select a winner. Remember the work is both graphic design and HTML/JavaScript (the landing page needs to be functional)

  I've resubmitted #2 just to show you the user experience and design. If I compare it with the one you chose.. haha! poor design! in #2 I thought of keeping video background, so that it'll prompt em to play video. Each section will appear, and will be focused on clicking button. (keeping it'll gave them option to change it any time if they went wrong at any stage)! if they choose a logo.. section with form will appear and users will be auto scrolled to there! anyways who gives a damn here

Hi...so in that case, the first layout of the screen will comprise only of the video and the 10 spots and may be the submit button? Since the rest seems to be follow-up action seems to be in progression. Thanks - regards...

  Hi...am unable to check 8 but 4 and 7 I liked too so anything i might have have come up with would have been similar. So its okay - i am not submitting any more entries and might as well take it as an opportunity to learn from other designers. Thanks - regards...

Guys please remember I need the form to appear after the logo is chosen then when the form is submitted. Form disappears and the Facebook invite should appear.
