I need help to change the URL of my website categories.

There are three categories TO change:

http://gjewelry.com/?cat=23 Change to: http://gjewelry.com/fashion-jewelry

http://gjewelry.com/?cat=1 Change to: http://gjewelry.com/post

http://gjewelry.com/?cat=20 Change to: http://gjewelry.com/Premier-Jewelry

Do not do a redirect. I need an 1 URL!

Second :

my "Reading Settings" not work.
All posts appear on the home page. I want only 5 posts per page.
