Contest Brief

This is a competition for the best "clickable prototype" for incoming hospital patient forms. This prototype is in reference to a future mobile application that will speed up the processing of patients into the hospital system. Briefly, the concept enables users (patients) to fill out hospital forms from their mobile, and then send this data to the hospital’s own server system. The patient form has been attached to this competition. What we require is a digital, clickable prototype of how this form would translate into a mobile application. Once the patient has successfully entered in all the data into the app, this information will be scanned via either a QR Code or a Barcode, so a wireframe depicting this transfer of information is essential. Finally, we will require an e-signature feature, so a wireframe highlighting this is also required.

It is crucial that the prototype can be used on our own mobile phones for demonstration.
