Hello, Can you tell me the general shape of the logo (square, round, rectangle, etc) approximate dimensions would be fantastic. Also on the Word document I downloaded, I did not see any terms and conditions or a place for client to sign. Am I missing something or do you want me to propose something? The download was only one page correct?

Hi there .. Would you like this just to be for printed use or will you be using this as a digital PDF fillable form on a computer or on line? I can build it either way for you or do one that can do both. No need to add all kinds of fillable fields if you are just going to be printing it.
