PLEASE SEE ATTACHED WORD DOC

I made mistake and put $20 instead of $15 but I will honor. (Is there any way to pay top 2 people $10 or no?)
*I need Excel sent to my email to see if enough space to write and I can use...

This will be used to write on not to type into
In case I ever WANT to type into it is it possible to make it so I can type into it nicely and click on check boxes and then print?

Any way I can have it that by clicking on Service (Striping, Asphalt Repair, Sealcoating, Concrete, Signage etc)
Materials needed for that job will be listed?
For instance if I am just going to do a Signage job by clicking on Signage all materials I need for signage will be listed to print and If I am going to do a Striping AND Signage job everything I need to bring for Striping and Signage will be listed?

Paper size is letter size (8.5"x11")

Please let me know if you do Calculations etc in Excel also - Always looking for good reliable help.
