Logo needed plus icons for wellness website in development. URL is at http://goo.gl/CIURfq

There is a placeholder logo ATM, but it is too similar to another.

Color palette same as URL.

I need.

1. Logo with strap line for menu. In color.
2. Logo for footer with strap line.
3. Logo without strap line.
4. 3 x icons for 'Book with us' section.
5. Icons for menu ... 'Register' Login etc. (there are 3 placeholder icons for this section at URL).

Name of the company is same as the URL logo.
