We would like to build a simple MsAccess application for Water Pipes Installation company (Logo is attached) that allows the user to enter customer information (one customer and his/her requested services in one screen) and print the customers and their services report (multiple customers (ID, Name and registration date) and all their services in one page) (details are attached)

The criteria for selecting the winning competition entry are as follows:
- 70% on the beauty of the interface screens
- 20% Professional building of the application.
- 10% to be user friendly

I encourage the programmers to get help by a graphic designer to make their screen impressive in design in order to win the contest. Do not rely only on your artistic sense. The beauty of the screen design is THE MAJOR criteria of the contest (70%)

Please don't waste your time in providing "strong" features of the application or strong programming. My concern is concentrated on the best design of the interfaces. I am looking for impressive looking screen, not a strong application. (use only Access features capabilities)

  I did not provide any screenshots. All what I gave are only description of my needs and a logo. You are free to choose the layout and design you like.
  My purpose of this contest is to get nice design for an Access application I am intended to build.
  The challenge of this task is to provide an impressive looking interfaces (Screens and reports) using the available tools, features and capabilities of MS Access

Participants need to have both skills (MsAccess Developing and Graphic Design) or to be done by a team of two. The Prize is 200$ to encourage this collective work.

The reason I chose to make it a contest is that the main concern of the project is the design of the interfaces (70%) not the software it self (it is a simple application). I want to have variant choices of interface designs.
