I already have a finished PDF and simply need it converted into an interactive, fill-in form. All the original layout, fonts, and design should remain intact; I only want to layer form functionality on top. Please add: • Text fields wherever someone needs to type responses • Check boxes for the relevant yes/no or multi-select items • Drop-down lists in the places where people must pick from a defined set of options At this stage I’m not certain which, if any, fields should be pre-populated, so build the file so that content can be easily added or left blank later without breaking the form. Deliverable: a fully tested, fillable PDF that works flawlessly in Adobe Acrobat Reader and in-browser PDF viewers on both desktop and mobile.
