The form should feature a 1-to-5 rating scale as the primary input, presented with an eye-catching yet unobtrusive UI that matches my existing design language. Key points • Single page or modal embed that drops neatly into my current HTML/CSS stack. • Visual polish matters: responsive layout, intuitive star or slider control for the 1-to-5 scale, clear typography, and accessible colour contrast. • Vanilla JavaScript or a lightweight framework is fine; avoid heavy libraries unless they genuinely improve UX. • On submit, data should post to an endpoint I specify (REST or simple email handler), with graceful success/error messaging. • Code must be well-commented so I can tweak labels or styling later. Acceptance criteria 1. Form renders consistently on desktop and mobile. 2. Rating value (1-5) and timestamp reach the endpoint in JSON. 3.
