We have been using the "ip2location" database (they update the db it frequently and they include city, region, and country) since 2018 internally in startup business's workflow I have been collaborating. For the purposes of this business it has been working fine. I am not sure if you have specific "accuracy standards" that have to me met. I can certainly leverage the ip2location database, it fits into CPU memory so the look-ups latency shoul be literally at 'speed of light' even if you were able to request the IP of 100000 email senders at the same time. Generating a weekly CSV report with IP-to-location data should be just a small code extension, as you can imagine. I understand you requirement is that the whole thing works like a plugin in your browser (desktop browser) but since you do not require any fancy UI, I am totally fine with that.
