webpage that prints barcode labels

I would like to have a webpage that a customer visits; enters in company information, shipping address, an alphanumeric SCAC code( a 4 digit code unique to that company) a starting number for the barcode; and the number of labels When they click on submit, the script would create a PDF file that would have a layout of a avery 5160 label, it would repeat each number 3 times. The number would have to have a mod 11 or mod 7 check digit also. The PDF file would be emailed to a static address to be printed up.
