Small ASP Project work to be done

Hello,
I need to get small ASP work to be done. The other programmer was already working on this project but due to some issues, he could not continue the work.
The requirement of the project is as follows:
The site offers members to fill the form asking various details (MANY fields in different sections), we have to provide FAQ for each of the for question.
For example:
If the Question is asked as
No. of children - Yes or No
We have to provide question for the field "No. of children" so if the question is difficult the question can click on the field and get the explanation about the asked question.
*** The example I gave is just a test example, the questions will be different. ***
2nd part of this module will be, we have to allow Admin to edit the questions answered by members.
3rd Part is - We will have to add option to email all the questions filled by user to the admin and user as MS Word attachment.
I need this thing to be done within this week. So i will close the bid in 2 days.
My max budget for this project is $300.
