Banner/Logo Fix Simple Design Job

I need our company logo cleaned up from the horrible photoshop job that I did to it. =)

The original logo's lines were too thin, so I tried to increase the size, which made it choppy and left globs in places. I just need it to be a nice clean image again, but with the same thickness/consistency, so it shows up well on the website. This should be a very simple job for an experienced designer.
I need the following formats when complete:
1. .PSD
2. Vector Image for printing (We want a large wall poster, so the bigger the better!)
3. Transparent GIF for Website
