Survey and Database

I have a simple survey. I have around 40 items in the list. I will send this to a mailing list I alreat own. I am asking those I send the query to, to review the list of items, fill out the form by selecting only 10 of those 40 items, and assign each item selected a priority number from 1-10, with 1 being the most important, 2 next, etc. When the form is filled out by an individual, I want that person's information to go into a database, that corresponds with the survey form, and collect their answers, and also the answers of all recipients. Then, we can know what the most important issues are to them, and develop our program marketing based upon the results. I know I can get a cheap survey form designed on fivver. But, the designers may or may not be able to develop the accompanying database to collect the information? I need both and I need it quickly and I need it inexpensively. What platforms(s) do you recommend to accomplish this task? What is your total price for doing this? You do not have to write questions or do anything except create the form and a method for data collection and analysis.
