We need to take paper registration forms and put them online.
We need there to be two types of users:
1. Regular users who,
a. register (does not have to be more than a username and password)
b. access (or REaccess) this form
c. Fill it out.
2. Administrative users who can
a. access any form
b. make changes to forms
c. print out one form or many forms
d. run reports
The data should be able to be saved at any point and recalled by that specific user for editing.
We can use MS SQL SERVER or MYSQL/PHP.
We need to be able to report on person by county (the county that the person who has registered is from). A list of cities and their corresponding counties will be provided.
We need to be able to report on city (the city that the person who has registered is from).
And maybe one or two additional reports.
