Template forms - Engineer Forms on iPad or Surface Pro

Please read and understand our requirements before submitting your bid: I need some ready to use template forms for use on tablet & mobile These forms are to be used by our engineers during maintenance of CCTV & Fire Alarm Systems. Ideally we would be able to edit the sections/content ourselves. Once completed our engineer would obtain a signature and email the completed form. Please note: This job isn’t data entry it is to design a simple interactive / dynamic form that can be used by our field engineers to compile reports - tick boxes, drop down boxes, insert text, insert photos, obtain customer signature and email reports. We will provide a draft layout with the required fields. We simply require the design and formatting of the forms
