I need a form for teachers to fill out that is in excel. Each area needs drop downs with the enclosed answers and then a comment field.
Along the top needs The name of the building, Field for childs name, what age, Teachers name field, Date, Student ID, Days Absent, and Quarter.
In the first column needs to be the Diffrent areas being graded Social and Emotional, Physical, Language, and Cognitive. And for each of these areas they need to be able to select from the spreadsheet of answers. They need to be able to add as many answers as they want and then for each area have a place that expands that allows users to write comments.
Then at the bottom they need to be able to have a big comment field that is the place a teacher writes the note to the parent. Then the user should be able to save and print this in a fashion that the parent can view this. THis can be multiple pages.
THe xls is the answers and what they relate to and the document is a sketch of what it might look like.
