Need an MS Excel template made to do the following.
1) intent is to place the computer in a public place, and have visitors sign in their all their info (first name, last name, home add, ph number, and e-mail). Need to have a header for the input box that I can edit... such as "Enter to win a XXX" OR "Registration for Oct 25 MSRH"
2) Need to be able to turn on and off some of the above fields easily. Meaning sometimes we only need name and e-mail.
3) Need the interface to be simple, with ONLY entry fields being editable to visitors, and a 'submit' button. The rest of the sheet would be locked, and NOT visible to users.
4) I need to be able to unlock the sheet, and save the list of names, add, etc as a regular excel spreadsheet.
5) This need to be very easy to use, as myself and the users are computer illiterate.
6) Need to provide detailed instructions for manipulating the spreadsheet. (how to turn off and on fields, how to unlock the sheet, how to save the list as a regular excel sheet, etc.)
