232829 Convert artwork

We need the attached artwork converted from a Word file to an eps file. There needs to be 4 files made. 1.) Back of the shirt approximate size of artwork 12.75" wide x 13.5" high 2.) Front of the t-shirt 4.5" tall x 5" wide 3.) right sleeve 3.5" x 3.5" 4.) Left Sleeve 3.5" x 3.5" The files need to be in eps format, original artwork. NEED ASAP!
