Fitness Card Template

My company will be placing wellness card in fitness center in both hotels, apartment buildings and work spaces and we need a clean easy design for roughly a 4x6 (no bigger than 5x7) card that contains workout information and sponsor information for the clients to take with them while in the facility.
These cards will be double sided with space one side with sponsor information or graphic images of the exercises or a QR code for scanning, essentially we will need image placeholders (or whatever idea you may have that is better suited for that).
On the flip side will be the workout. I have attached a pdf with a very rough version of what the workout card may look like. Please be creative and play around with the design, I only included this pdf so that you may see what type of content I need to be included. I would like to see clean, fun, hip design, be creative. I also included the Emergence logo for your use. When we place these card in the hotels and workspaces I would like to be able to use the respective location's logo, in place of the emergence logo. Essentially they will be customized for the specific client, so please keep this in mind during creation.
I would like the final product to be a template that is able to be edited online via a cloud editor as we will be changing these each month.
