CD an CD cover artwork, graphics needed.

I need a CD label and CD case artwork designed for my product to match my website design. The CD case would be the large size ones that you typically see DVD movies sold in. See attachments for examples.
The CD case would be for the front, spine and back of the case. This should be designed as one piece that would slide into the case and fold with it on the outside. Again, see attachments for examples.
This artwork will be used to create printed CD’s and CD covers, so keep this in mind for the size of the artwork.
I would also like 3 graphics that would represent the CD and case if it were photographed. The 3 graphics would be for use as an example on my website and I would like it to represent the different positions represented in the examples.
Please note: I do NOT want the artwork to be styled like the software box graphic on my website. The software box will be replaced by the graphics you create. Only use the style of the website itself including the header. Text can be the important features stated on the website such as the bullet points or other text we agree on.
