Formal Word Doc Template Creation

My company requires a tender response template created in word that we can use for submission of tender responses, project briefs and formal proposals. There are a number of simple business proposal templates available in microsoft word which are simple and professional, however we would like something that includes our logo and colour schemes, that looks sleek and professional with a well considered graphic design. It does not need to be complex, however it will need a professional cover page, customised header and footer, and a contents page. I will provide to the successful applicant a copy of our current letter template, and also a template from another company that sets the benchmark for what I require. We will likely use the same freelancer again in the future for some minor graphic design of finalised tender responses and proposals prior to submission.
