I need an experienced, licensed nurse to come to my home and provide consistent, compassionate care that focuses on two areas: managing a chronic condition—diabetes—and helping with everyday activities. The clinical side of the role centers on diabetes care: checking and recording blood-glucose levels, administering insulin, spotting early signs of hypo- or hyper-glycemia, and offering practical education on diet, activity, and medication timing. Alongside that, I also need gentle daily assistance: help with bathing, dressing, light meal preparation aligned with diabetic guidelines, and basic mobility support so day-to-day life remains safe and comfortable. Good communication with both me and my primary physician is essential, so clear visit notes and timely updates are expected. Deliverables • Initial in-home assessment and written care plan • Visit reports after each session (vitals, glucose readings, observations, recommendations) • Monthly summary for my doctor, including any supply needs or medication issues If you have proven in-home nursing experience with diabetes management and a warm bedside manner, I’d love to discuss scheduling and start dates.
