I am a pharmacist and i need to design a simple MS Access database with a simple Data Entry Form for the Patien Tests performed daily , a Print Form for the test results , and a reporting form with date selection
In the future if needed we will adjust or add features
I need this to be ready quite soon and as i do not have much free time , and i do not have MS Access knowledge i need outside Help
I experimented with a template of MS Access which i will upload as to see what i need.
We have the Data Entry Form.
1. User will enter patient details on the upper part
2. We need default data to appear on "Manufacturer" , "Model" , "Pharmacy" , "User" but user will be able to alter them if needed

4. Only ONE TickBox "Positive" or "Negative"
5. All information should be stored in the database
6. Then we need to print the info in Half A4 paper with background the included "FORM" and the data mapped as explained in the marked sections in the included "FORM-FIELDS"
7. We need a REPORT that Prints a List of the tests done for a period , Displaying the following Data :
Also because there are going to be repeated visits from same patients , i am thinking of a separate table with patient data , so the user can search an "ARC" to find the patient and then by selecting him the form would come up with all data filled for quicker entry
