Job Description:

The web application should have the following features and functionalities:

• The index page should have a child (student) registration form and login form already registered students.

• Registered students will enter the sport page to select a sport such as Soccer, Basketball, Baseball and Rugby etc.

• After selecting a sport, student will complete the registration with giving information such as number of shirts and number of shorts.

• Students will checkout by giving a credit /debit card details to make a payment. You don’t need to store the credit card information in a database table.

• After successful payment, a confirmation message will be displayed in a page.

• Student will view the profile page for personal details, the history page for registered sports.

• Student will modify their personal details in the profile page like address, city, phone number, doctor information etc.

• Provide a friendly and easy to navigate web pages with the use images and CSS formatting or boot straps.

• Create MySQL database with following tables (Student, Sport and Registration) with fields and apply primary key and foreign key constraints. You are free to add any missing appropriate fields in the following tables.
