Website Template update & form creation

Hi,
I have picked up a free website template from one of the website and modified few elements on the same, attached are original HTML files along with updated designs. I want you to pick this up and create a complete the website (3 Different templates) which includes
1. Update existing HTMLs or Create new HTML (What ever your are comfortable)
2. Creating drop-down menu as shown in design (Not available in original design)
3. Create a form as per design and upon filling this form capture the data filed and send it to a given email id. (No DB required)
I will be hosting on Windows server hence pages should be in .NET only
Some one from Mumbai preferable.
