Book Formatting for InDesign

Anywhere from 100 to 300 pages changes the quote, and Kindle reflow versus a fixed paperback layout need separate passes in InDesign, not one export. Is the manuscript in Word with heading styles set, or still raw text? That decides how much cleanup comes before layout. Send the file and I'll say how I'd split the two passes.
