Relocation Policy Form Design (Power point and PDF)

We are looking for a freelancer to design a professional, branded Relocation Policy & Financial Modeling Intake Form for City Shift Finance. Deliverables: A polished, fillable PDF form that clients can complete and return. (with brand colors and logo) The editable source file (PowerPoint, Word, or InDesign) so I can make future text or branding changes without needing to rehire. Form Requirements: Clean, corporate design that aligns with relocation/finance services. Logical flow: company profile, relocation scope, benefits/coverage, policy guardrails, financial inputs, strategic goals. Easy to complete with text boxes, checkboxes, and dropdowns where appropriate. Professional but user-friendly layout. Experience preferred: Prior experience designing intake forms, surveys, or corporate questionnaires. Strong skills in PowerPoint, Adobe InDesign, or similar tools. Ability to advise on layout for best client experience.
