Embedded PDF Modifications in PHP

I have an embedded PDF in PHP. The PDF format currently exists and needs to have 2 things adsded to it. #1 - The current graphic layout needs to be moved. #2 - A. Depending on what check box is highlighted there needs to be either of 2 PDF Pages added to the begining of the PDF File. B. The either of the 2 pages needs to pull the text from the .txt files that have the data in them. C. The PDF File then needs to pull data (Name, Street, Zipcode, Agent) from a database file and add a date to it. This job requires an expert in PDF's in PHP. If you are not an expert please do not bid.
