Simple Survey Site

I need a simple site built (no fancy graphics - ican provide graphics) to let an administrator create a simple survey (based on a matrix). The end user would click a link on a web site and answer a series of multiple choice questions using checkboxes, radio buttons, or drop down lists. Based on this feedback, the site would present a filtered down result set of matching items. Here is an example: Administrator would build a small survey to help a user choose a printer. System would have a list of all printers in a SQL table along with features that are checked YES or NO. Example: LASER, INKJET, COLOR, B&W, CHEAP EPSON X X X CANON X X X X LEXMARK X X X IBM X X MINOLTA X X X So at this point, the user would be asked a series of questions. 1) Do you want a Laser or Inkjet Printer? User would choose Laser. So immediately, the site knows that only these three printers apply: LASER, INKJET, COLOR, B&W, CHEAP EPSON X X X IBM X X MINOLTA X X X Next, the user is asked: Do you want to print Color? He answers yes. So the following printer is the recommended choice: LASER, INKJET, COLOR, B&W, CHEAP MINOLTA X X X Hopefully this will give you an idea. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform ASP with Javascript on Client and Server Preferred.
