I require an online tracking tool writing.
The database will require 2 tables :
1) Operator. This will solely consist of a list of names, along with a flag stating if the Operator is active/inactive.
2) Appointments. Each time an appointment is taken the following information will be recorded :
- Operator (drop down box populated with the list of active Operators taken from the Operator table)
- Address 1
- Address 2
- Telephone Number
3) Payments. Consisting of :
- Payment Amount
- Method of Payment
A form will be required allowing the site admin to enter one of the following to return a list of all matching appointments :
Another form will be required showing the list of all (both active and inactive) operators that when submitted will return a list (in date order) of all all appointments listed and all payments entered, giving an up to date "outstanding balance".
