Facebook Authentication added to MySQL Table

I need to utilize Facebook's authentication feature on my application's so that users can join my website by logging in through Facebook. Once they've allowed my app to access their information, I'd like a unique identification number, first name, last name, email address, current location (their 'current location' on Facebook, not their actual geographic location), gender, and relationship status added to a MySQL table. I need you to write the code that allows users to create an account with Facebook, prompts them to accept permissions, stores information in my database, and redirects them to a new page, as seen in this tutorial ([login to view URL]) If you can leave the App ID and Secret ID blank, as well as the database information in the code blank, I can update that information into the code and verify the information has been added in the database. I can also create the table in MySQL. Thanks!
