Yes, we can detect IP address and can restrict the user from giving the exam more than once. when user will click start button the questions will show through ajax (and a timer will start?). after end of the exam the answers will auto appear? or there will be a button from where you can control that the answer will show or not? I will give you exactly what you want even if it takes more time.

Hello Sir, My name is Andrew and I think we can get this project completed right away. To construct the page and the scripts should only take a couple of hours. I have a few comments and questions before we get started: Do you already have style sheets to apply or will the CSS need to be developed as well? Is the use of jQuery acceptable? Doing so will make development faster for me, specifically for the AJAX, and make for easier maintenance down the road, but I can easily use pure javascript if you prefer. Do you want to incorporate some of the scripts you have already downloaded, or are custom scripts acceptable? Some kind of user detection should be possible, perhaps in layers. IP detection is doable, although not very secure. Most people have dynamically allocated IPs, and could be coming to your site multiple times with different IP addresses. Another option could be as simple as dropping a simple cookie onto their computer marking that they have already taken the survey. A skilled user would be able to delete this cookie, and anyone who clears their cookies or turns off cookies would break the system too. I'm not sure how much anonymity you are aiming for, but a third option could be to ask for an email address in order to complete the survey. All of those safeguards are possible to circumvent, although average users won't know how or won't care. I believe these are the best tracking options short of creating accounts. Let me know if this will work for you.
