I have a PHP form that uses data in a MySQL table to craete a PDF document. I want to change the form and the PDF output, so that different data will be extracted, and will be presented differently in the PDF document. Very urgent, need to start right away.

I'd be happy to take a look at this for you, assuming that the existing script you have in place already generates a PDF file similar to what you're looking for and would only require some basic modifications. I have more than 5 years of experience in PHP, and have done several projects which export PDF content (some with HTML2PDF, others using a variety of PHP-native PDF libraries).
