HTML form with validation & MySQL

Create a dynamic HTML form with data validation (PHP, Javascript, Ajax) , entry into a MySQL database and sending of a confirmation email to user & admin. Features of the form: • Main form: max. 30 form fields (text ,dropdown, radio, checkbox & file upload) • Subforms: only toggle or inserted as a step if applicable with max 14 form fields each • max. 6 hidden fields • form should be divided in steps to avoid scrolling • editable by user without any loss of data • info icon with speech bubbles • show summary of entries before user hits send button • create a unique processing id number • create folder for every process • validate, rename and move uploaded files in folder • create a [login to view URL] file and move in same folder • move an [login to view URL] in same folder • parameters for validation must be easy to configure by admin • login data for MySQL will be implemented via ‘require’ • form size should be optimized for tablet pc resolution (max 700 px wide) • every form object with CSS ID
