simple appointment script

I need a script for an online appointment workforce/resources scheduling system which will be used as a hosted service for multiple clients.

This script should be written in PHP, and MUST use MySQL Database (NOT Microsoft SQL). The web design should be professional, up to date, and customizable with tables and css. I need to own the full license and copyright of this script without any limitations. The selected programmer will need to provide me with at least 30 days of unpaid script debugging and support after the script completion. The selected programmer should be good with email dependent scripts because this script depends on a good backend email system. The selected programmer would also have the benefit of being hired for more paid projects in the future for this script.

Programmer(s) MUST:

- Accept payment via Escrow or Paypal

- Provide daily project progress report

- Be easy to work with

- Be very professional

- Have the ability to create and prepare the script as I see fit

- Be available 30 days after script completion and receiving full payment. For unpaid script debugging and support (just in case)

- Provide a full demo before releasing payment
