235849 PHP/MySQL Form

I need a php page edited so that each of the options have the correct input fields as describe by the requirements below. The user will be selecting one of the options. The options they select should be saved in a database (one table that you create/design) so when the user accesses this page again, they see the options that they selected previously. When the user clicks on "generate code" for each option, the form code needed is generated so they can copy/paste it onto their own web page. Attached is the correct form code for "generic" - you will need to create the code for the other options based on the particular payment method given i the requirements below. The requirements for each option are below. If you complete this job properly, I will have a follow up job which involves integrating the generated code with the order placement code. If validation succeeds then the Order form will be displayed, only one order per purchase will be allowed. For clickbank only one combination of cbreceipt+time+item+cbpop will be allowed and must be unique 5.) If all required fields are completed the form will post to our system that will then redirect the successful order to either the (optional) Thankyou Page the customer provided or to our Generic Successful Order page. (Notes with 1shoppingcart, if all the information is there the order should be directly placed with us sending the customer directly to the Thankyou page 100% automation is possible) Generic 1.) Ask For the follow information a.) If the Order is completed successfully an (optional) Thankyou Page 2.) Completion of Question will present our Generic Form. 3.) The Order form will be displayed, only one order per session will be allowed. 5.) If all required fields are completed the form will post to our system that will then redirect the successful order to either the (optional) Thankyou Page the customer provided or to our Generic Successful Order page. PayPal 1.) Display the IPN URL and a link to the training video on how to set up PayPal IPN on their account eBay 1.) Ask if they are using PayPal as their payment option for eBay. If yes, display the PayPal IPN URL. If no, display the Generic options (above). Amazon 1.) Amazon order validation should be completed using curl (their API is too complex). 2.) Order validation is automatic (the customer does not get a choice). If the order is valid, the information curled from Amazon is posted to our system that will then redirect the successful order to either the (optional) Thankyou Page the customer provided or to our Generic Successful Order page. 3.) If the order is invalid, the information is posted to our system but with a status of "Invalid Order". A notification is placed on the Home page so that when the user logs on, they get a notification right away that an invalid order has been attempted. 4.) You need to add page validation error checking to the attached curl script. This should notify the system administrators that the Amazon page has changed so the script needs to be updated.
