My client has an urgent need for a database, which will list volunteers available for recruitment. The interface should consist of two screens - inputting and viewing (searching).
The database will allow manual input of an individual contact, including his/her name, address, phone numbers, email address, notes about the contact, availability, whether the client fits certain 'Specific Requirements', and what skills they have.
All personal details (name address etc.) should be free text fields on input, displaying as read-only text on the search screen. The 'Specific Requirements' options will be tick boxes, and the 'skill' list will be a drop-down list. On the input screen, users should be able to add options to this drop-down list via a free text input field.
Once the information is inputted in this manner, it should be searchable on the view (search) screen. The search screen should have filtering options for skill (via the same drop down list as the input screen), 'Specific Criteria' tick boxes, Availability tick boxes, and a text search for name, address, phone number etc.
This is a small project, so I'm looking for a quick turnaround and low cost. I've put MYSQL as the database, but I have no real preference. I don't want a solution that requires proprietory software, and I need something that will work on Windows and Mac.
If whoever wins this bid does a good job, there are two other pending projects listed on my profile, which the successful candidate will be well considered for.
Please see enclosed screenshots to get an idea of the layout I'm looking for. As long as the solution has all of the options shown on these screenshots, I'm not too bothered about aesthetics. There will be a company logo (jpeg) which must be shown on both screens. This will be supplied to the bid winner.
