Customer Database

I need a way to manage my current and future customer database. Currently, I am using Microsoft XL (spreadsheet).

What I would like to have is a page (such as [url removed, login to view]) where I can add customer info. It would look something like this:

Name

Address

City

Zip Code

Phone

Comments

etc.......

Then, there will be about 10 or so check boxes. Each check box has a value.... such as "Completed Job" or "Paid on time" or "Paid Late" etc...

Then, there needs to be a search function. Where I can search by any of the fileds (name OR address OR Zip code OR PAID LATE).

For this project, I will also need my current XL database converted into the MySQL database.
