Outsource CGI Form

I need a form that has multiple drop down boxes with options. When you pick a different option I need a text box at the bottom (and maybe the top) to change to a new total. I then need a script (I would prefer a free one that you know of to keep costs down) that will take all the fields selected and write it to a basic HTML page that I can customize (ALL I NEED FROM YOU IS A WHITE BACKGROUND PAGE WITH THE VARIABLES). It would also be great if I could take that TOTAL value and move it into a shopping cart price box to that will then move this value into the Checkout page. I really just need a description on how this might be done since I am not sure which shopping cart software I will be using. So to recap. I basically just need a basic form (that the values can be changed... I will do all that jazz), a script to move the data to another HTML page and then a description on how I can put the total into a shopping cart price text box. I also need the form sent as an email to both seller and buyer telling what options and the final price, ideally on the second page so they aren't making mistakes and sending multiple copies. This is just a small part of a project I am doing and I just don't have the time to figure out how to do all this. If you can comment everything and it is very easy to use, I will probably send you a bonus when I get paid. I don't expect this to be very difficult for someone with more CGI experience than I have. I will need this done in 2-3 days. I will accept the first bid that I am confident that can do it an is in the $50 range. Please leave a private message if you need more details.
